The Investigation of Structural Failures in Ships - A Paper by Jeremy C Colman
Structural failures in ships can be massively damaging to life, property and the environment. There is a duty on Naval Architects, regulators and operators to carry out thorough investigations into such failures and ensuring that the lessons learnt are recognised and reliable. This paper discusses methodologies for reliable investigation into structural failure, the tools available to the investigator and the skills required.
 
The Loss of the MFV Gaul
This paper sets out to describe one of the most extensive investigations ever into the loss of a fishing vessel, the MFV Gaul, lost with all hands in 1974. The background to the paper lies in the original Formal Investigation held in 1974. Following location and subsequent surveys of the wreck, the Investigation was re-opened in 2004.
